Donatometer problems

Sry I am having to ask this once again. I had my hard drive go out and lost everything as well as I have had this block installed and working once before. I have seen in various places this block is working for many, the download has been done and the instructions on the install are not at least to me, to where is easily understood, as well as if an sql file is needed it is not included in the download. Can someone please help me in where to get the sql file if needed and a little more detailed instruction installing nuke treasury donatometer like is here on this site.
