Poll/Survey Submit redirects to another domain?

I have a subdomain:

http://www.gastricbypasslongtermliving.com

on my main domain:

http://www.bobmiedema.com

Both sites are running 7.6 patch level 29

For some reason when I try to submit a vote in a poll on the subdomain website, the submit button redirects to my main domain poll box....

Of course the POLL on my main domain works fine

Any idea what could be causing this? Everything else works fine on each site, this is the ONLY fault that I can find.

NOTE:

I had redone both websites just a couple of weeks ago with FRESH installs of v7.6 and then my HOST was down for 6 days and my sites have only been online since Sunday.

Try editing the blocks/block-Survey.php
Find this line and try adding ./ before modules.php
$content = "<form action=\"surveys.html\" method=\"post\">";

IF that doesn't work try adding $nukeurl to the globals line and using ".$nukeurl." before modules.php
