voting help request

I'm using nuke 7.6 and my club would like the ability to perform online voting for the board member election.

question 1. Any suggestions on a method to do online voting by registered club members only?

question 2. is it possible to have a voting form if you will.. that has the candidates listed and have either a check mark or bullet to fill in AND have a text field to enter your name and email address?

Nice idea.
When you say 'registered club members' - are you using some form of 'groups' permission system?

Yes sorry I didn't include that piece of info.. the registered club members get assigned to a group. This group has access to member only forum content.

If there is a voting system out there that work off the homepage where I could tie in this group or a new group for members access to the voting system would be ideal.
