Remove your IP from the nsnst_blocked_ips table (using phpMyAdmin) and if your IP is listed in your root .htaccess file you will need to remove it there as well.

What happens when you run the installer? Are you getting a blank page or are you getting an error?

I too have experienced this issue. The short urls work when manually entered in the browser, but do not get automatically rewritten. Not sure how to proceed to correct it.
