Are you sure you have the right lang-english.php file up there? Could you list here in a code block say from lines 107 to 113 of the file you are using, or just try to re-ftp the file from the distribution and see if the problem goes away. If there was a wrong parameter count in the distributed file then everyone would be seeing that error.

Check also your captcha within the block it looks like the old security code
