Popup calendar

Trying to find a good calendar popup to use on a php form for choosing dates. Anybody know where I can find a nice simple one that is easy to implement. I have downloaded a number of them from http://www.dynamicdrive.com but none seem to work.

In the large index.html file just take this part....

and put the above in place of your input text.

Next to the input field a little icon calendar will show and when you move your mouse over it the window shows...
You can choose from day,year.month....
