Survey Block Not Working Anymore

My Survey block use to work a long time ago then I updated my phpnuke (bad idea) and had to revert back to 7.6, don't ask why I updated we'll just say I wasn't thinking. Well for the time being I just deactivated the block and left it at that. But now I want the block to work again. When someone goes to cast a vote in the survey the person will get the following error.

Also I don't think I can even change the survey question anymore.

Any ideas? also sorry if this is the wrong section XD

Is your version Patched or standard phpNuke?

Well before it stopped working I was just running 7.6 and went to 7.9 not knowing. So I deleted everything and reinstalled 7.6 and the survey never worked since. Then I installed NukeSentinel 5months ago. Thats about all I have done.
